Most of my perimenopausal patients are happy, when their periods are gone . And I understand them: no spotting, no bloating, no tension, no unwanted pregnancies, etc. So now they can take hormones to get relief hot flashes, anxiety, sleep loss, mood swings and get their youth back. Looks like everybody is happy:
- women, because they look and feel young again
- doctors, because menopause management is this way.
But not any more. The recent Danish study results proved 2 things:
1. Risk of heart disease was lower, if women were cycling.
2. Transdermal route was superior.
These finding are supporting previously very well know point of view, that cycling is better. So if you want your youth back, get some inconvenience. But you will be rewarded.

Are you concerned with your cholesterol? Yes, of course, because if it is high, then your risk of heart disease is increased. So what needs to be done, if you want to bring your cholesterol down? Go to your doctor! So you go and what do you get? A cholesterol lowering pill, of course. Are you happy now? Yes! Because the only thing you need to do now is to take pills. Simple? Yes! Hormone replacement therapy is very effective for menopause symptoms relief. Therefore when HRT was first introduced, everybody was very excited about it: patients, because HRT worked very well, doctors, because now treatment of menopause became easy.
Then Women’s Health Initiative study results came out. They found that hormone replacement (HRT) increased risks of heart disease, and breast cancer. Hormones, used in WHI, were conjugated ( horse) estrogens and progestins ( not natural progesterone). The other statistics confirm these finding. For example they found that breast cancer rate in the US is declining because of cutting down on HRT. So what is the problem with traditional HRT?

The problem with synthetic or semisynthetic hormones is utilization: our body does not know what to do with them. But our body knows exactly what to do with our own hormones. Example: natural estrogen is metabolized by 2-OH pathway, which creates benign waste products. Conjugated ( horse) estrogen is metabolized by 4-OH pathway, which is not natural. The result: byproducts, linked to cancer.
Then doctors came up with the idea to use hormones chemically identical to our own hormones: method known as bioidentical hormone replacement. In fact these hormones have been on market for a long time. Example — Prometrium ( bioidentical progesterone). Estradiol and Estriol have been well known Biest. However of bioidentical hormones used in famous WHI study. Why? Because nobody wants to put money into the medications which cannot be patented. Have you seen Pepsi advertising? Many times. How about broccoli? I do not think so. Same thing with hormones.
As a physician I’m concerned with safety first. Remember: do no harm - ancient wisdom. Are bioidentical hormones safe? Do we have controlled double blind studies to confirm safety and effectiveness of the female hormones with the same molecular structure as our own? No, and we won’t have - at least in the near future. Because substances in nature, cannot be patented. Therefore no money to subsidize the studies.
Do we really need controlled studies? Effectiveness of insulin was confirmed on patient after he was injected with pancreatic extract and his blood sugar level decreased. Power of vaccination was confirmed on 21 patients only. No double blind studies. Would we get insulin or vaccines, if double blind studies were required?
Our experience and our colleges experience show, that hormones with the same molecular structures as our own are effective and safe for hormone replacement therapy, if used correctly under appropriately trained physician’s supervision. I know, that they are working for menopause and andropause. The new study, done by Aksam Yassin, MD, PhD from the Clinic of Urology and Andrology of Segeberger Clinics in Norderstedt, Germany, confirmed, that testosterone replacement therapy does not cause prostate cancer. Do we have to wait until controlled studies come out?

If somebody asked me this question 10 years ago, I’d consider that person crazy. But not now. Because from my personal experience as a physician I know, that a low sugar level is very dangerous. And this was confirmed in a recent study, published in the New England Journal of Medicine this year.

Do you have back pain? I did. And it was not the best part of my life because back pain is horrible. It prevents a person from sleeping, working and relaxing.
What is the most common cause of back pain? Too much excess weight! Think about it: the more pressure you put on your spine, the faster it breaks down. It’s the same as your car: the more you use it, the faster it breaks down. So what needs to be done for back pain?

Everybody was surprised, then saw Angelina Jolie on October 4 2008 after she gave birth to twins. Why? Because she was slim again 3 month after delivery. There is no question, that she looked fabulous. The question is how she could do it.

Do you want to have your periods back? Yes? No? Most of the women say no. And it is understandable: who wants to have this trouble again. If you decide to get bioidentical hormone replacement, you may have it without periods. And everybody loves it: youth is back but no periods, no pregnancy. Another choice is to go back to cycling with periods again. There is no consensus which is better. And I usually recommended my patients BHRT without cycling. Until recently.
